Whether your child is learning in the classroom or on-line this school year, Generations Family Health Center’s Putnam School-Based Health Center is here to help you and your child.

The Putnam School Based Health Center (SBHC) is a partnership between Generations Family Health Center and Putnam Public Schools providing dental and behavioral health services to students right at their schools or via phone or video if they are learning from home this school year.

Who is eligible?

All Putnam Public School Students Pre-K through 12th grade.

What services are available this year?

The SBHC is providing behavioral health services both in person and via telehealth during the 2020-2021 school year.

Telehealth therapy services include telephone and video telehealth visits. Telehealth is easy and safe to use from the comfort of your home. The therapist texts or emails a link to your child to click, and they are connected via a confidential, HIPAA-compliant connection

Due to the pandemic, dental services are not available at the School Based Health Center. We are scheduling children at our dental clinic at our Putnam site at 202 Pomfret Street.

  • Putnam Middle School-35 Wicker Street Hours: Monday – Friday 7:30 am to 4:00 pm FOR INFORMATION OR APPOINTMENTS FOR ELEMENTARY OR MIDDLE SCHOOL STUDENTS CALL 860-928-4698.
  • Putnam High School-152 Woodstock Avenue Hours: Monday – Friday 7:30 am to 4:00 pm FOR INFORMATION OR APPOINTMENTS FOR HIGH SCHOOL STUDENTS CALL 860-928-2714.

For Dental appointments:

How do we get services?

Please click on the links below to complete the REQUIRED registration paperwork. Your child must be registered at the beginning of each school year to be seen. The registration paperwork can easily be filled out and submitted on a computer or smartphone. Forms with an asterisk* are required.

This form must be completed to consent to treatment for your child.

This form must be completed to confirm that you received a Notice of GFHC Privacy Practices (attached above for your review).

BH Agreement between Outpatient and Patient or Parent/Guardian:

This form must be completed and signed by the child and parent/guardian. It informs the child about the Behavioral Health Department and their rights as patients.

Ongoing Communication of Protected Health Information – SBHC:

Please complete this form to give us permission to communicate with someone other than the child’s parent/guardian. This might include a relative, the school nurse, or someone else involved in your child’s care. You may also use this form to restrict information that is released to anyone.

  • The School-Based Health Center accepts most insurances. For those students with insurance, the insurance will be billed.
  • Eligible families may apply for a sliding fee scale discount, which could decrease your co-pay, based on family size and income.
  • For the uninsured, we will work with families to screen for insurance eligibility. We will help you apply for a sliding fee discount and work with you on a payment plan.



We will be communicating with you about the care your child has received and if there are any recommendations for further care. However, you are welcome to contact us with questions at any time.

Safety for In-Person Appointments:

Our number one priority has and will always be the safety of our patients and families, our staff, and our community. All staff of the SBHC and students in care will be required to wear a mask, utilize hand sanitizer prior to the visit beginning, and remain 6 feet apart at all times. A plexiglass germ shield will be between the student and therapist.

Medical services:

If your child is in need of a medical provider, we will be glad to assist you in setting up an appointment at our Putnam office, located at 202 Pomfret Street, Putnam, CT.

Are behavioral health services available for parents or others in the household?

Yes, therapy services are also available with our Putnam outpatient therapists for parents and others in the household to help cope with stress and other effects of the pandemic. For more information, call 860-963-7917 (press option 4) or go to:
